Ranked Queue Bug. Got a loss in series never getting ranked queue or champion select pop up.

Somehow I managed to get a loss in a ranked series, never even having the chance to accept queue. I was sitting in queue for 7 mins two times in a row. So i Logged out thinking logging back in might get me a game. I come back and I have been marked as losing a game in my series. I never Had the chance to accept a game, let alone dodge in champion select. Does someone want to explain to me what happened?

The same thing happened to me earlier this year. I was in que for ranked when one of my friends logged on. I declined the match because I wanted to play with my friend and it gave me a loss in my series.

On a totally unrelated note, I was playing a custom game as Kalista just myself against bots because I wanted to try her out and see how she worked. After I got a feel for her I left the game about 10 minutes in, it lost me my promos to silver.
