DIANA'S PASSIVE AND Q R DON'T ALWAYS WORK

In every game as of 5.13 i've encountered a problem where diana's passive would not go off after 2 stacks and it would just repeat 2 stacks for 2-3 more auto attacks. also, her q r reset has not worked in one of my games, the target was clearly marked and there was no chance it had warn off since i had just casted the q and the r reset did not work.
