Lost my summorner

My summoner name was godadog

with the new patch i forgot my login name. I got a new login name and then had to get a new password bc what i thought it was wasn't working.

i put a new summoner name in when I finally was able to login. hadanamelostit.

how do i get my summoner back that I spent so many hours on?
