Randomly within the same game or you load into a game and it's 50/50 there? Need more specifics.

does toggling health bars in the settings do anything in either of those those cases?

Below is only if toggling the settings does something: You might be hitting the shortcut key sometime during the game. If so, just unbind it I guess and see if that stop it from occurring.
