[Crash] Twisted Treeline causing PC to lock up

I tried out a game of Twisted Treeline today with my girlfriend. All was going well until the end of the game when we won. Upon clicking the continue button that takes you back to the score page my entire computer locked up. I couldn't even move the mouse and had to suffocate my computer to restart. I tried it again and the same thing happened, as soon as I click continue to go to score screen, my computer locks up every time. Not sure exactly what kind of information you'll need for this issue, I've made a DXDiag report but am unable to post it in this board but I'll provide any information that you require.

It was Intermediate Bots 3v3 on Twisted Treeline and happened both games despite having entirely different champions.
