Forced dodge without missing ban or champion select

Some friends and I were in champion select when one of us dodged as if they didn't ban or select a champion but it wasn't even their turn to pick or ban. He was given a 6 min que, but he didn't dodge.

It's only happened once, but I'd rather not see that again.
