bug report in champ select

always get a bug report in champ select and i click send error and it prevents me from picking a champion or maybe banning one therefore loosing league points and getting a 30 minutes punishment waiting in the lobby this occurs every single game only in champion select if anyone can help me i will be thankfull :(
