Jhin Ultimate Bug

There is a huge jhin ultimate bug that has been going on for a day or so. Its causing instant death in game and its terrible. I can get a image or video clip but its horrid to play with and it happens only in Nexus Blitz I think. It fires all four at once or does a two-stage kind of ultimate.
