[GamePlay] Rek'Sai Applies Black Cleaver to Herself

I noticed this happening after I bought the deaths dance the damage prolonged taken from the item might have something to do with it. It Only Happens when You use her Queens Wrath (Q). Anyways watch the video and if someone could explain I would be grateful.
