Hot/Quick keys not working

This issue is crippling to my game play i lose every time this happens. My Hot/Quick keys are failing to issue my ability commands, active item commands, and summoner spells. This persists for 5 or 6 games every time this happens and then magically goes away. I have tried using one of the forums ideas to delete something from the LOL file on my computer and not re-installing the game. It seemed to work but in about a week and a half the problem still persisted for me.

Important points

1. This inst champion specific i have found that it happens to all of my champions.

2. It seems pretty random as to when it happens, and when it does show like i said it stays for awhile.

3. I've tried replicating it on purpose to find more information so riot can fix it, but i can not do it.
