Yesterday I have COOP for the first win, and of course I win, but I didn't get my first win reward. And the next time I try to get 150 bonus, I suddenly found it already been taken. eh, Where is my first win bonus.
And today, when I try to play ARAM, at the count down,it stuck on the champion screen for more than 5mins, and I found that someone left the room. I quit forcely, however I get a punishment for 10mins. That is unfair to me....

Sometimes the client will bug out due to lag or whatever other reasons and the timer will show that you still have it when you don't. Relogging will fix it. You can look at your Match History page in your profile and see if you got extra IP.
As for your leaver penalty, if someone else had left then you wouldn't have gotten a penalty for leaving. Most likely you were the one that had left. Looks like you have a lag problem or something.
Things like this is why they are overhauling the client to get rid of the spaghetti code.
