Well, an enemy Xin came at our Tristina, and when she ulted, he kinda flew from the mid lane to the bottom lane, which was awkward. So yeah, in a team fight Xin dashed at Tristina and when she ulted, he flew crazy high and across the map.
It is one of the oldest bugs out there. I have had it happen to me numerous times on almost any champion with a dash. It is about 50% reproducible. Just dash at tristana, and if she activates buster shot while you are in motion, she launches you halfway across the map, usually in the direction of the middle of the map.

Well I didn't have a Buster Shot bug in my game against Tristy, but when I played as Quinn and Vaulted on her while she Rocket Jumped away, I ended up getting carried with her, and landed in the direction she was facing. So basically Vault functioned properly, but because of the dash aspect of it, I ended up in a really weird spot.
There's tons of videos on this on youtube. it's a known bug.
