Karthus ult cd

The icon in the bottom right corner indicating if your teammates ults are up restarts for karthus's icon if karthus ults successfully before dying then dies later before the ult comes off cool down looking like he just used it. This occurred on howling abyss. This made it look like he ulted when he died instead of earlier.
