Your group has been disbanded

I was queueing for a game with a friend and we finally got into champ select. Aside from the chat not being able to be used, neither of us having a summoner icon and him not even being added as a friend everything seemed fine. However, mid way through champ select the group was disbanded even though no one dodged as we were not placed back into queue. What is this?
