"Didn't Ban"
So I was in a lobby for ranked and it said the last pick on their team was banning and it got down to zero for a very long time then it kicked me because it said I didn't ban but it wasn't even my turn to ban.. what the hell

That's you disconnecting from champ select so when it gets to your turn you get booted.
Just happened to me as well. Counter timed to 0 for the enemy ban, nothing happened for about 10 seconds, then it rapidly showed the ban and started my timer. I banned, then I started seeing "player x left the room" massages and I was back in the team assembly menu with a dodge penalty.
