missing mastery 7 token

I had two mastery level 7 vayne tokens, and now i have one??? Also i just got an S+ and didn't get a token??? Why was one of my tokens removed????????

The S+ you got recently was on Twisted Treeline, which is why you did not get a token. Tokens can only be earned on normal and ranked SR, you will not get them while playing ARAM, Twisted Treeline, or any of the rotating game modes.

As for why one of your tokens was removed, I don't know.
