This happens when 
dies mid dash because when using Q 
are grayed out. This started to happened at the First URF 2016, but i know i started noticing it more in regular games. To solve this i need to use q again. Plz fix this its so annoying
If you die mid Q this bug happens, it's super annoying. With my ~220k points in Fiora, I notice this bug wayyyy too much, at least once/twice every match.
Oh also, I'd like to share the build that I'm pretty sure only I and the #5 Fiora in the world are using (Aegis EnGarde)

Runes-> attack speed quints ad marks armor yellows scaling CDR Blues
Take fervor keystone
