Sometimes Cant get into matches

Sometimes when i queue for a match,and i get the "Match Found" thing,after clicking accept,and after everyone accepts,i'm returned to the homepage,but it still shows that i'm in the champion select and i can hear the music,i cant do anything to go in.And because of that i got banned from future games ,i mean that time penalty for like 8 times.
