(Gameplay) Katerina Bug, unable to use skills until death

Recently I have encountered a bug with katerina. It has happened to me twice in the past week and usually occurs when doing fast combos involving most of her skills. After the combo I am unable to cast certain abilities, in both instances my Q wouldnt go off anymore. The first time i was unable to base, Q, or E. This bug has always reset after a death. However this is a real annoyance, having to give a kill over, just to be able to do anything again.

Im sorry i dont know exactly what causes the issue but i hope some others have also found this bug and together we can form a good image of whats wrong so i can be fixed.

when it happens i usually execute myself to jungle

usually when you are getting a reset and you que up the skills in fast succesion. idk exactly but when i go QEWWE or something sometimes they lock, but id much rather execute to a jungle camp then give a free kill to the enemy... It has happened a handful of times to me

Iv'e reported this issue on several occasions and Riot does nothing just ignores it
