Elementalist Lux bug

This problem is with Elementalist Lux and her form indicator, As we all know Elementalist Lux can change forms after hitting champions with her abilities (the progress of which is indicated above her character icon). In patch 9.4 her form indicator is missing from its original position and is nowhere to be found, however after landing her abilities and maxing her form gauge the indicator appears to let you choose a form after which it disappears again until you fill it up again and reach your final form it goes away afterwards and never returns. (this problem happened in all the games I played during patch 9.4)
