Three Error Bugs

The three images attached describe the three bugs I keep encountering while playing LOL on the NA servers. The first image being the fact that every first attempt to login produces this error screen in which I have to reopen the client and do everything again. The second image is of two classic Vel'Koz skins that appear to have remain throughout some updates. The final image is of black box Gnar tag where his name is supposed to be.
