[GAMEPLAY]Fiora's passive not disappearing

Time to time it happens to me that a vital shows on enemy, and when they are out of my vision, vital stay on them instead of dissapearing but another one shows as well. However only on of them is a real vital and u can't trigger the second one with your auto, but you don't know which one is real.
