Graves bugs

I've recently begun playing Graves and have encountered a few bugs/graphic errors. The main bug I'm having is his w. A few times while casting 2 abilities at the same time it seems that the w has just disappeared? For instance, I was fighting trist in their base, launch my w, q and auto later realizing my w was never there yet it was on cool down. Has anyone else experienced this? Also, near the end of the game my auto's turned into my ult animation for some time, it was strange, possibly just occurring for me.
