[CLIENT] Says I declined a game or left during champ select even though I didn't.

There is no particular set of steps I can describe to duplicate the problem. During normal use of the client, sometimes the client will just kick me out of a champ select queue and tell me that I left during champion select. Also, sometimes when I click ACCEPT, the animation will hold for several seconds then tell me that I declined the match (even though I accepted it). Any reason why this might be happening?
