Patch 9.3 Spear of Shojin

So patch 9.3 came out and I bought the item Spear of Shojin on Shen and the item** DID NOT** activate when i Q Drag plus E combo after i ulted my ally. I thought Shen Q's are auto attacks. If Shen Q's are not auto attack why does Shen misses if Teemo Q's blind Shen.
