Evelyn E and lich bane not working 100% of the time

Eve's e is supposed to trigger and proc lich bane, butt it doesnt do it 100% of the time, sometimes it will just do the base damage leaving you with an unused lich bane proc on your active effects bar and only doing effectively half the damage.
