I que for ranked, patiently waiting... Accept Que when prompt shows, a player failed to accept, 3 more different prompts show up before all players accept the game, and when I finally get in, it instantly crashes after the screen changes and makes a sound. There goes another 5 minutes of wait time and 3 LP?

Shouldn't there be like a 10 second timer to verify if you got in? I understand why the dodging punishment is there, and I totally agree with it, but allow at least 10 seconds for your client running 6 years + to figure whether or not it's crashed itself please.

Assume that it's a bug that like telling Mother Nature to give us a week warning before we get a Tornado. Besides any double check method could be abused to dodge. Simplest method is, Are they in? If no then punish, if yes continue as planned. Besides if you want Riot to figure out if it's a bug give them more info then I crashed. Perhaps your OS and OS version, if you're running the lastest drivers. If you've tested the client for corrupted files, if it's happened more then once, if you can force it to happen, and etc etc. Otherwise just expect this post to be removed for useless/wrong boards.
