Remake failure to fire

Has anyone else experienced a game where their ADC or some other player connects, buys items, then shortly thereafter disconnects, never to return? Of those games, how many of them have failed to end in a quick 'remake' vote once the timer hits 3 minutes?

It's incredibly disheartening to lose a game that could've easily been avoided if the Remake feature activated properly.
