Died for no reason

Hello, unfortunately I have no replay client so i cannot be sure, but based on the other player agreeing with me, I died for no reason. This was post team fight and i was clearing out a ward with low health, when I randomly started taking damage from a W. was nowhere in the area and said she did not use W, nor did i see it. I have never had a problem with a bug in the many years I've been playing, so seeing one doesn't bother me, I would just like for, if there is one, for it to be patched (or at least looked into) so this may never happen to me or any other players again. As i said I cannot view the replay, so for some reason if this was not a glitch I apologize. This game took place on 9/16/15, was a ranked game, I was playing as , and occurred around 4:50 EST. This death occurred to the northwest of the purple team blue, and took place somewhere between 20:00 and 25:00, (I believe around 22:00). Thank you for reading this and I hope when someone has time that they can look into this, and if on the off chance this happens again, I will add a comment or something along those lines informing you of it. I hope I am wrong as I'm sure it isn't fun to scan through the code for bugs, but better safe than sorry.
