[VISUAL] Incorrect information/bad visuals in Death Recap

I've noticed two recurring bugs in the Death Recap screen. Please see the link for pictures.

First (and most often), my own summoner name will be listed as one of the causes of death, usually with a picture of a tower or a minion.

Second, sometimes all three of the "causes of death" will be pushed on top of each other, rendering all of them unreadable.

Both of these were captured in ARAM.
