Yea, that's a bug with Yorick and his ultimate. I don't know the exact specifics of the bug, but I know that it exists due to my friend mentioning it.

It's just a Yorick bug. You know how his ult revives someone, yes? If the temporary revive timer runs out, the game deals 100000 true damage to them to kill them off again. That part is normal. The problem is that it's attributing the damage to the revived person, so they're both giving and taking 100000 true damage when that happens.
