bug in champ select

There was a bug in my most recent game in champ select. I wasn't able to swap champions with a teammate. I first picked yasuo, and he picked ammumu. We communicated in chat, the intention to swap. I sent a request, and nothing happened. It said the trade request went through, and he doesn't swap. I get mad until in chat, we both start talking about the swap. Apparently both of us sent a request, and neither of us got it. It doesn't seem like this is a very common bug, but it just cost me 14 lp with amumu mid and yas jungle. Can someone take a look at it please. I would imagine this kind of bug would be very crushing in higher elo.
