Map smudging?

This happens a lot, i think it may be because i have two screens and i tab out occasionally, but many times i load up a game and see something like this, sometimes in different places. looks like texture smudging to me, when i walk through it the bottom half of my chracter is covered by it, it's an eye sore at most and not a huge deal, just not working as intended
