Video Example of Fiora Ultimate Bug [GAMEPLAY]

I'm posting this view that I had of the Fiora Ultimate bug in the hope that it may help shed some light on its cause.

In this example, Fiora is not very close to the target as many have suggested may cause the bug (it may be different entirely, or purely visual in this case). Most recently, Fiora has struck TF with the second tick of her E. The shatter particle effects attached to TF play as does the sound, but the zone itself does not shatter, visually at least.

Unfortunately I am unable to shed light on the steps to reproduce this particular bug. In fact, this is the first time I've seen it manifest in this way.

By my estimation from recent play, this bug occurs every 2-3 games while playing Fiora.
