Player Failed to Select a Champion Bug

This has been for a while, and I don't know if this is a bug or what, but every second game (and I am not exaggerating) it will happen, that mid-way ranked champion selection, the game will be disbanded and we will get a message saying " a player failed to select a champ". Now I'm trying to figure out WHO THA HELL failed to select a champion?!? The timer isn't at zero and in most cases people are picking their champ, so it would be locked in when the timer ends. In other instances it even happens when everyone has already picked a champ, and it is the last 30 or 15 sec for you to pick spells and other stuff, you know switch champs, and it is gonna suddenly disband the game saying a player failed to select a champion. How Yomayo? Everyone already picked?!? Now I want to note, this is about a player failing to select a champ, not leaving, if somebody left there should be a message saying player this and this left the game, but it just says failed to select a champ. Now I think it's a bug, but if it's not, then I'd like for the system to be less ambiguous about the player and at least indicate who left, then at least I'll know that sb failed to select a champ, and if it is say one of my friends who didn't leave, then I can really report it as a bug. P.S. Speaking of thing to fix, the bottom line extras are really getting in the way of writing...
