Zed Bug?

I have talked to many people while playing Normal Summoner's Rift matches, and have found out that some people, while playing Zed , also have an issue of getting their Blade of the Ruined King (BORK) to go off. I have found a ok fix by moving the item to a different slot and readjusting the keybinds, but i would like the keep my original keybinds and not have to adjust to new ones. Please let me know if this is a bug, glitch, or something. It happens right after you ult as well.
