So Yasuo's Q is counted as a Basic attack, so it can critically strike and apply on hit effects. But how come on hit effects such as say, Wit's end don't apply to all targets hit, instead of just the first target hit? I understand that it may be kind of ridiculous, but it just makes sense, since all enemies hit can be critically striked. It is understandable if say a Sheen only proced on the first target hit though.

Its exactly how it sounds. It counts as a basic attack. So if you have hurricane and have sheen proc, the hurricane shouldn't proc the sheen. It would be way too strong. So the Q applies on hit effect like sheen on the first target cause THAT is the first minion or champion hit. The others are just attacked by the regular Q because the on-hit effect was removed after the first proc

The tool tip in Yasuo's Q specifically says that it only applies on-hit effects to the first target hit. That is why on-hit effects won't apply to targets beyond the first. Frozen Mallet should not be applying to all targets hit, and neither should Wit's End, due to this limitation. If they are, then it is a bug.

Black Cleaver on the other hand, is not applied specifically by basic attacks, it is applied when you deal physical damage to a champion, so Yasuo's Q will apply Black Cleaver stacks to all targets hit.

It only procs on first target hit, to avoid abuse (triforce)
