Sounds like a classic case of the 2D game meeting 3D visuals. There is no height in the game, so bushes are visually taller than their real hitbox. When viewed from a low angle especially, visually tall objects may cover some area higher than their hitbox really does. This is why you can have a ward behind the grass on top of a bush but it's not really in it, and why a lot of skillshots go wide when it looked like a hit.
If you could see the true hitbox of everything it'd be very clear if it's a hit or not, but to make that 100% accurate in the game we'd be seeing a bunch of boxes and nothing in 3D.
