Vi after death able to use all abilities except Q?

I'm sorry I don't have a video or pictures for proof, but I will do my best to describe it. The link above is my match history of one of the 3 times this occurred if you can somehow watch game footage from my point of view (I'm pretty noob at this stuff). Anyways 3 times now while playing as Vi ingame I was unable to use Q at all not even activate it but my other abilities were just fine. I again could use my E ability so I binded my Q ability to my E key and tried to use it but still it wouldn't work. I switched back to Q and it again still wouldn't work. These instances always seemed to follow after dying while either silenced or hooked by a blitz or thresh for instance. I'm very sorry I don't have much more info I hope there is something that can be done or help show this error/bug that is occurring if it is happening to more than just me.
