Kicked out of promo for no reason

I was on my diamond 4 promo, the first game, I won, the second one I lost, and the third one I won, when I finished the third one I qualified to do the d4 promo again instead of passing to d4. I'm pretty sure I never dodge during the promos or anything, can someone please help me with this.
