Two issues that I can't seem to solve

I've been having two huge problems of late regarding my ranked games that I can't seem to solve:

A) For some reason, I tend to disconnect at least once every ranked game. I have 4g net, a gaming laptop with up-to-date protections etc., and I've cleaned out my whole system and have run anti-virus programs to my heart's content to no avail. Is anyone else experiencing a similar issue? How did you fix it?

B) Sometimes when I disconnect I can't use any items, abilities etc. after reconnecting. They're just gray icons that resemble those of used items. Needless to say this is a massive problem in-game where I can't do anything but use my basic attacks and emotes. I can't use anything else and it's extremely frustrating.
