I got 150 orange shards + a permament teemo skin, sadly i didn't have teemo at the time and after i purchased him the skin was missing. The image is for reference (i didn't expect it to glitch like that so i didn't screencap the whole thing, this was for my friend to see).
The picture you shared is a summoner icon, check your summoner icons.
I have a bunch of skin permanents in my hextech inventory for champions that i don't own yet. They should sit in your skins loot until you get the champion, then it is free to forge it, once you've gotten the champion and it is activateable. That screenshot does look like you got a summoner icon and it might be confusion on your part- check your icons to see if that icon is there and if so, you might have just gotten the icon and not the skin.
