[CLIENT] Can't get to champion select screen

Recently whenever I try to play ranked I can't properly get into the champion select screen without something getting messed up. After I click on Accept the screen is stuck and I go nowhere. At this point one of two things happen, either I go back to the home screen and then get penalized for five minutes for dodging (even though I didn't) or this happens (I've attached a picture). What the hell should I do to solve this?
