Issue with disconnected from Champion Select while the game continues on, counting as a dodge

Occurs around 2 out of every 10 games, and happened moments ago for a friend while we were in Champion Select while I was in placements so it counts as a loss and a -3LP penalty for him. Is there any way to prevent this from happening or is it Riot-related?
