Kayn Transformation bug

I have played many Kayn games since his release, and in at least 1/3rd of them I have not been able to transform. The meter fills up like normal, however the description of the transformations disappears and the portrait becomes unclickable. After disconnecting and reconnecting, it changes back to the original kayn portrait with no options to transform at all. VERY frustrating bug. I have played gamed and been able to transform, so i know the conditions that must be met and how to transform. Why release a champion with a bug that doesn't let you access his main abilities? Ive included screenshots that were taken at the end of the game and on the fountain so that all conditions are obviously met to transform at this point.
