[GAMEPLAY] Katarina suddenly loses abilities

So I was playing Katarina just now and something strange happened. At some point in the game, I can't use my Bouncing Blades, Death Lotus, and Recall button. I don't know why, but I can recall what happened leading up to that. It seems it all started after the first time I attempted to use my ult.

1. In mid lane against enemy Teemo with 2 of my other teamates
2. Throw Bouncing Blade to Teemo
3. Immediately use Shunpo to Teemo
4. Immediately use Sinister Steel
5. Immediately use Death Lotus
6. Does not get the kill. Only assist. Teammate gets credit for kill.
7. Cannot use Bouncing Blades, Death Lotus, or Recall

Furthermore, I took note of the fact that the new passive ability, Voracity, in which 15 seconds is refreshed after the death of an enemy champion if Katarina deals damage to him/her within 3 seconds, did not take in effect. My cooldown was at its full timer. It was immediately after this that I realized I couldn't use my Bouncing Blades. Every time I try, she just walks up to the target. Furthermore, I eventually realized that I can't recall as well. At some point, I had an enemy champion test my ult out to see if it works, and she doesn't activate it at all when in range of the enemy. Katarina just runs up to the target as if I intended to auto attack it. The same goes for Bouncing Blades.

I don't know if this has been a recurring issue or it's because of the new patch to her, but I do not want to play her just yet as it has been disappointing being pretty much useless that game. In effect, I haven't tried to reproduce it. I hope this can be fixed soon.
