[Gameplay] Yorick ult and quinn ult after quinn dies means permanent valor

We were playing ARAM today 7/25/2015 on NA servers. I was playing as quinn and another player on my team was yorick. The steps to cause the bug were as follows:

1. Team fight starts
2. They got me, or so I thought
3. Yorick Ulted me
4. I want payback so I ult and go on their adc's and die
5. I am in Stuck as valor(the bird) the rest of the game

-I tried to ult later in the game to change back but it never let me go back. Sometimes I could shoot arrows out of valor. Sometimes I couldn't.
