Summoner Name Bug In Champ Select

Unfortunately I do not have a screen shot for I have noticed the bug too late.

So I was in Champ Select and was put in the Mid Role. Then one of my teammates with "X" name asked if I would like to switch roles. When I looked my right to check it was P3 with the Jungle role next to name "X". So I picked a jungler and locked in. It wasn't until 2 seconds left during the game count down did I notice that P1 had the exact same name and he was actually going Top. So I now our team had 2 junglers. I decided not to waste the other players' time so I decided to quit and force a remake. This was extremely unfortunate and inconvenient as it was during my promos, and I had to take the loss. It was extremely weird and I thought that perhaps they were premade and that I mixed up 2 similar names. However, afterwords when I checked their names outside of game, I realized that P3 had and completely different name. I honestly don't know if I was just blind but to have not noticed after looking at their names for about 4 mins, I don't think I was mistaken that they had the exact same name in Champ Select and it really threw me off.
