Client Crashes during champ select

My client has crashed during champ select twice in draft and once in ranked. Windows reports that the client has stopped working. When I log back in it goes to the main screen. Once I try to join a queue, a message pops up saying "A server error has occured, ..."

Then I can join a queue, but have to wait 5 minutes to join as if I dodged..

This is getting very frustrating, as I am getting punished for something out of my control.

Has anyone else had this problem? Is it known? And if not what should I submit to help?
