WE HAVE POSSIBLE DATA CORRUPTION DETECTED PATCH 6.16.16_08_12_05_46

Hi Guys. I have probles with that bug or whatever it is. When i play Normal or Ranked games, in champion select i pick my champ, lock in and when the game starts (it showes up loading screen with champs) i have that bug. My loading screen shuts down and i have some pop out window with ziggs picture and it says Somethings Broken and button to Send Report or Dont Send. And when i Send it, i cant reconnect to the game, I must exit the game, then Laucnh again, and when i do that i have on the screen those words: We have detected possible data corruption in your instalation.... And when i click ok then hes updating patch and when hes on 11-12% i can launch again game... and then when i come back, the game has been finished (remacked) and i have in Match Histroy - Leaver.... can some1 please help me with this problem i dunno what to do... I reinstalled my windows, League of Legends too... I changed my password on acc too... And that bug i have on 1 of 5 games...
